With towering NHS waiting lists, many trans men or transmasculine people are choosing to get their top surgery done privately. This is usually rather expensive, but how much does it really cost?

What is top surgery?

Top surgery, also known as a bilateral or double mastectomy, is a gender-affirming surgical procedure where a person's chest is reconstructed to alleviate gender dysphoria.¹

While a majority of trans people seek to undergo top surgery as part of their transitions, some others may not see it as necessary. In other cases, it may simply be too expensive to be viable.

How much does top surgery cost on the NHS?

While the NHS is generally considered to be free for people ordinarily resident in the UK, you may have to pay for treatment if you are a visitor from abroad.² For UK residents, the NHS is funded through general taxation of the population³ with approximately 20% of your total tax paid going towards funding health services.⁴ This means that you still have to pay a nominal fee for access to healthcare—gender-affirming or otherwise—but this is not dependent on the actual cost of your care to the NHS; this system is commonly labelled as "free at the point of use".

How much does private top surgery cost?

A number of private clinics in the UK offer top surgery and similar gender-affirming procedures for a fee. While most clinics do not publish their prices online—instead recommending that patients phone them to talk through their particular situation before obtaining a quote—a small handful provide rough estimates that can help in determining how much to set aside if you're looking to undergo the procedure.

In some cases, people with health insurance may find that gender-affirming care is a part of their coverage, bringing the cost down significantly.⁵ The total cost in this case will usually be dependent on a patient's excess alongside their usual premiums. However, top surgery, HRT, and even gender dysphoria diagnoses are now covered by fewer health insurance providers than they were even a few years ago.

One clinic in the UK, Pall Mall Medical, offers masculinizing top surgery from £9,995, though the cost in more complex cases can easily exceed this by three to four thousand pounds.⁶ They also warn that the cost of top surgery is not limited to the fees for the surgery itself, but can also come in the form of prescription charges—for painkillers, for instance—and the cost of supporting equipment during the recovery period—like compression tops and special wedge-shaped pillows for sleeping on.

Another clinic, The London Transgender Clinic, has a detailed price list outlining starting costs for a variety of surgeries directed towards transgender people.⁷ Initial consultations cost about £250, which is a common figure for clinics across the country, and the surgery itself can start from £8,915 for surgeries via a peri-areolar incision (where an incision is made around the nipple⁸) and £12,415 for full double mastectomies plus liposuction.

Due to the high cost of treatment, many clinics offer payment plans, though the interest rates on these may be high. Any form of credit like this must be carefully considered against your financial situation, and optimally you should consult a finance professional.
